The Efficacy of Directional Atherectomy Prior to Stent Implantation

Nonrandomized studies has shown that plaque removal prior to stent implantation using DCA is a promising approach to reduce restenosis. This complex approach could be used in lesions at high risk for restenosis in whom efficient debulking is feasible (noncalcified lesions in vessels greater than 2.75 mm but less than 3.5 mm in diameter): 1) lesions that require a long stent or multiple stents; 2) aorto- ostial lesions; 3) bifurcational lesions; 4) chronic total occlusions.

In this observational multicenter registry, 120 patients had angiographic follow-up. The overall restenosis rate was 10.8% (13/120). Restenosis was lower (8.4%) in patients with optimal stent deployment following DCA compared to patients with a persisting caliber reduction >15% (restenosis rate 15.3.%). Finally, restenosis occurred with a significantly higher frequency (p<0.04) in distal lesions (37.5%) compared to proximal stenoses (9.0%).  Comment from www.tctmd.com Click here

In this study, 176 successful stented CTOs were retrospectively analyzed. After successful crossing of the lesion, 50 lesions underwent debulking by ELCA, Rotablator or DCA followed by stenting, whereas 126 lesions underwent stent implantation without prior debulking. MACE were similar for the two groups. At a mean 14 +/- 8 months follow-up time, there were no deaths in either group, and TLR rates were the same (16.3% in the debulking plus stent group vs. 14.4% in the stent alone group, p = NS).

Of 406 randomized patients, 377 underwent attempted atherectomy, and in 324 (88.9%), a </=50% diameter stenosis was achieved without the use of a stent. Evaluable follow-up angiography was available in 292 eligible patients (90%). No differences in MLD (1.99+/-0.73 mm vs 2.00+/-0.74 mm) and angiographic restenosis rate (23.4% versus 23.9%) between the placebo and carvedilol groups were observed at 7 months.
