The aggressive rotational atherectomy strategy offers no advantage over more routine burr sizing plus routine angioplasty. Operator technique reflected by an rpm decrease of >5,000 from baseline is associated with CK-MB elevation and restenosis.

Despite many years of widespread application of this device, technique of Rotablator is still very subjective and changing. Moreover, the efficacy of this device is highly operator dependent compared with other devices. Although the Rotablator could not be the mainstay of percutaneous coronary intervention, the Rotablator is indispensable device for specific lesion unsuitable for POBA and stent.

At seven clinical sites 502 patients with pre-defined complex coronary artery lesions were assigned to either PTCA (n=250) or rotablation (n=252). At 6 months, there was a comparable symptomatic outcome, target vessel reinterventions and restenosis rates between the 2 strategies (PTCA 51% versus rotablation 49%, P=0.33).
