The results using the Cutting Balloon for the treatment of in-stent restenosis may be superior to those of conventional PTCA or even the combination of PTCA preceded by rotational atherectomy. The reasons for these possible differences are not yet well defined. The case we report suggests that the Cutting Balloon achieves a better final result than conventional PTCA, by making the tissue more amenable to being pushed outward through the stent struts.

Role of cutting balloon angioplasty before coronary stent implantation

One hundred and seven consecutive patients with 120 lesions scheduled for elective coronary stenting were randomly assigned to either a cutting balloon group (group 1)or a conventional balloon group (group 2). Angiographic follow-up was obtained in 105 patients (88.9%of those eligible),including 50 patients (84.7%)in group 1 and 55 patients (93.2%)in group 2, and the rates of angiographic restenosis were 20.0%in group 1, and 20.0%in group 2 (P =not significant). In conclusions This study shows that cutting balloon angioplasty before stenting may not be effective for the prevention of in-stent restenosis or major clinical events after coronary stent implantation.
