This study demonstrates the predominant role of lesion and procedural factors in
determining the occurrence of restenosis after coronary stent placement. Among
these factors, stent design appears to play a particularly important role in the
hyperplastic response of the vessel

The primary end point of target vessel failure at 9 months was seen in 15.1% of
ML-treated patients versus 16.7% of PS-treated patients, with the ML proving to
be equal or superior to the PS stent (p <0.001 by test for equivalency). In a
prespecified subset, angiographic restudy showed a nonsignificant trend for
reduced restenosis (ML 16.0% vs PS 22.1%).

The primary end point of target vessel failure at 9 months was seen in 16.0% of
NIR versus 17.2% of PS patients, with the NIR proving to be equal or superior to
the PS stent (p <0.001 by test for equivalency). Angiographic restudy in 71%
of a prespecified cohort showed no significant difference in restenosis (NIR
19.3% vs PS 22.4%).

Five types of stainless steel stents were randomly compared: MULTI-LINK, NIR,
Palmaz-Schatz, PURA-A and Inflow stent. The incidence of restenosis varied
between 25.3% and 35.9% (P = 0.145). Thus, stent design has a significant impact
on the long-term results after coronary stent placement.

Stent longitudinal flexibility: a

Stiffness, the reciprocal of flexibility, was measured for 13 stent designs in
the unexpanded (bare) state, then after expansion with a 3.5-mm balloon.
Stiffness was least for the coil (Wiktor and Crossflex) and hybrid (AVE GFX and
Bard XT) designs. The MultiLink was the most flexible and the Crown the stiffest
of the slotted tube designs. All stents became stiffer upon expansion.

performed in 1998-99. At six-month follow-up, there was one sudden death (3%)
and 3 patients (10%) had anginal complaints (one of them had TLR)

In this study, 317 consecutive patients treated in 1997-98 by Multilink stents
were retrospectively evaluated. A control angiogram was performed in 94% of the
patients at 178 +/- 34 days: restenosis occurred in 19.4% of the lesions.

RACECAR Study
- Restenosis and Clinical Evaluation in Coronary Arteries
6-month angiographic F-up results with the Medtronic AVE S670 Coronary Stent
System
RACECAR is a multicenter, prospective, nonrandomized trial designed to evaluate
the clinical efficacy of the sixth-generation S670 modular stent design in
patients with unstable angina pectoris and de novo lesions in native vessels.
Study enrollment totaled 267 patients drawn from 22 hospitals within Europe.
Conclusion: The RACECAR study was successful in demonstrating that the
Medtronic AVE S670 stent system had a high procedural success rate (97 percent)
with few acute (3.0 percent) and long-term (8.2 percent) complications, while
providing a low incidence of cumulative six-month TLR (9.0 percent) and
angiographic (9.9 percent) restenosis.
