Chronic total coronary occlusions were more frequently crossed using the Crosswire as a primary guidewire strategy than with the conventional
strategy. This strategy resulted in a lower number of guidewires being used, a trend toward shorter procedural and fluoroscopy
times, and decreased use of contrast media.

Report on the results of a nonrandomized single-center investigation using a hydrophilic-coated guidewire
(Terumo Crosswire). Between 1996 and 1998, 107 chronic occlusions in 106 patients were approached when previous attempts with conventional guidewires
failed. There were no deaths or Q-wave myocardial infarctions; two cases of hemopericardium were treated
successfully. In five cases, pericardial contrast staining due to vessel perforation
occurred. This results indicate that the Crosswire is an effective tool in the treatment of chronic coronary
occlusions, even when recanalization attempts with conventional guidewires
fail.

In this study, 176 successful stented chronic total occlusions were retrospectively
analyzed. After successful crossing of the lesion, 50 lesions underwent debulking by
ELCA, Rotablator or DCA followed by stenting, whereas 126 lesions underwent stent implantation without prior
debulking. MACE were similar for the two groups. At a mean 14 +/- 8 months follow-up time, there were no deaths in either
group, and TLR rates were the same (16.3% in the debulking plus stent group vs. 14.4% in the stent alone
group, p = NS).

A multiple logistic regression analysis revealed that the presence of
calcification, the length of the occlusion and the presence of multivessel disease were independent predictors of procedural
failure. Cardiac death and the need for coronary surgery were significantly less frequent in patients with procedural success than in those with procedural
failure.

flow. A considerable fraction of collateral flow was immediately lost after
recanalization, indicating that TCO may not remain protected from future ischemic events by a well-developed collateral
