Despite the use of new interventional devices, SVG interventions are associated with significant morbidity and mortality; SVG stenting is not associated with better three-year event-free survival. This may be due to progressive disease at nonstented sites.

Local abciximab delivery before percutaneous intervention for de novo SVG stenoses followed by intravenous infusion is associated with significantly reduced thrombus burden, significantly improved percent diameter stenosis, and excellent acute procedural results.

Comparison of the outcome of patients with SVG disease treated with polytetrafluoroethylene (PTFE)-covered stents or noncovered stents. Angiographic success was similar; non-Q-wave myocardial infarction was lower in the PTFE group (p = 0.06) and long- term major cardiac events and restenosis rate were similar in the 2 groups. Implantation of PTFE-covered stent in vein graft disease seems to reduce the occurrence of distal embolization

Multicenter study on 109 consecutive patients  who received polytetrafluoroethylene (PTFE) membrane-covered stents for 125 de novo stenoses in vein grafts 11+/-5 years after bypass surgery. One patient suffered from subacute stent thrombosis. Six-month cardiac mortality was 7% (8 patients), 3 patients (3%) underwent repeat bypass surgery, and 9 patients (8%) required target-lesion PTCA. Repeat angiography revealed vessel occlusions in 9% and in-stent restenosis in 8% of patients by the end of follow-up.

Treatment of SVG lesions continues to be associated with a high incidence of myocardial infarction, particularly in cases of degenerated SVG lesions. An effective blood supply to the anterior wall and a higher left ventricular ejection fraction were protective for the occurrence of death during the follow-up period.

Unstable angina caused by restenosis presents in as many as one fourth of patients undergoing PTCA of SVG. Dyslipidemia strongly, and to a lesser extent postprocedural residual stenosis, predicts its occurrence. Scrupulous attention to these modifiable risk factors may help reduce the incidence of unstable angina after SVG angioplasty. 

Late-Breaking Clinical Trial 
  SAFER (Saphenous Vein Graft Angioplasty Free of Emboli Randomized Trial) 
  Baim DS   Presented at the TCT 2000 Meeting
The SAFER Trial (Evaluation of the Clinical Safety and Efficacy of the PercuSurge GuardWire as compared to a conventional guidewire in Saphenous Vein Graft Intervention) demonstrated that the use of a distal protection catheter device reduces the risk of cardiac events associated with SVG stenting. The PercuSurge Guardwire System, which evacuates blood and debris caught behind an occluding balloon after stent placement, also produced improvements in TIMI flow. The study is a "pivotal" investigation for clarifying the role of distal protection devices.
